NOTE: You are still required to return a completed and signed application form/waiver to the club before you can compete in any club events.

 I Acknowledge that I have voluntarily applied to Ventura Surf Club to participate in surfing contests and other club activities at different locations throughout the year.
I am aware that surfing is a hazardous activity. I am voluntarily participating in these activities with knowledge of the danger involved, and hereby agree to any and all risks of injury or death. As consideration for being permitted by Ventura Surf Club or one of its affiliated organizations to participate in these activities and use their facilities, I hereby agree that I, my assignees, heirs, distributes, guardians and legal representatives will not make a claim against, sue or attach the property of Ventura Surf Club, its Board of Directors or any of its affiliated organizations for damage resulting from the negligence or other acts however caused, by any of its affiliated organizations as a result of my participation in surfing. I hereby release Ventura Surf Club, its Board of Directors and any of its affiliated organizations from all actions, claims, or demands that I, my assignees, heirs, distributes, guardians and legal representatives now have or may hereafter have for injury, damage or death resulting from my participation in Ventura Surf Club activities.
I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Ventura Surf Club, its Board of Directors and/or its affiliated organizations and sign of my free will.
